By filling out this form, you are registering to attend the ITINERARIES MIDWEST GROUP TRAVEL SHOW


This most unusual group travel show is produced for tour operators, travel agents, bank club directors, qualified group leaders, meeting and corporate travel professionals, educators, field trip organizers, and other travel industry professionals. Because of the exclusive nature of this Show, FAM, & Transportation, there is a charge for group travel buyers to attend this 4-day event (see below for details.) Please fill out this Registration Form completely so that we may prepare accordingly.

In order to insure the best buyer/supplier ratio, registration will be limited to first come, first served.

IMPORTANT:
Itineraries Midwest will alert you when we have received your registration and payment. Accommodation confirmation, room assignment and pertinent show information will be sent to you approximately two weeks prior to the Travel Show.

SHOW PRICING & PAYMENT - Based on the number of people in your organization planning to attend (as shown above):
1 Person: $95 ($85 if paid before 07/15/2009)
  Includes all show events, transportation to Winnipeg, FAM Tour, & one hotel room for each night.
1 Person: $35 (without accommodations)
  Includes all show events, FAM Tours & meals.
2 Persons: $135 ($120 if paid before 07/15/2009)
  Includes all show events, transportation to Winnipeg, FAM Tour, & one hotel room for each night.
2 Persons: $70 (without accommodations)
  Includes all show events, FAM Tours & meals.

REMINDER: To cross the border you & your guest(s) will need either a current passport or passport card.
Passport Will allow travel to any country.
Good for 10 years.
Cost: $100 for first time Applicants
$75 for renewal
Passport Card: Will grant entrance to Canada, Mexico, the Caribbean, & Bermuda.
Not valid for air travel.
Good for 10 years.
Cost: $45 for first time applicants
$20 for renewal

It is recommended that you apply for your passport or passport card at least 4-6 weeks before departure time.

Cancellation/Refund Policy:
Cancellations made before x/x/xx will receive a full refund. Cancellations after x/x/xx will be credited for future shows.
